Documents Needed For Live Scan
Applicants without paperwork will be asked to reschedule appointment when the proper forms are provided.
- Request for Live Scan Services form Provided by the Employing agency. Please Make sure the Form is Fully filled out prier to the appointment.
- Acceptable form of Photo identification; Which include a current and valid: California Drivers License, California Department of Motor Vehicles identification card, or out of state drivers license.
- If applicant is under the age of 16 and does not have an acceptable primary form of photo identification, a birth certificate must be provided as well as the applicant having parental supervision or guardian present.
